2026 Veteran Banner Program Application

  1. The City of Beavercreek’s Veteran Banner Program is intended to honor the service men and women of the City of Beavercreek. To qualify for a banner, honorees must meet the following requirements:

    • A veteran of the United States Armed Forces, or sacrificed their life while serving; AND
    • Is a current resident of the City of Beavercreek, or Beavercreek is the individual’s hometown; OR
    • An immediate family member is a City of Beavercreek resident (i.e., parent, spouse, child, sibling, grandchild, child-in-law).
    • Active-duty service members are not currently eligible for this program due to limited number of display locations.

    The banner will include the exact spelling and information included in this section. 

  4. Please use the proper abbreviation as it should appear on the banner.

  5. Please choose one due to limited spacing on the banner. If none, put n/a. 

  6. The picture should be of the veteran in uniform or dress attire. High Res digital preferred (40" wide at 100dpi, approximately 5mb or larger)

  7. Verification of military status (Military ID or DD214 form)

  8. Your application is not officially received until payment is made for the veteran banner. Payment options include: 

    • Check/Money Order: $100 payable to the City of Beavercreek. Drop off at Beavercreek City Hall, 1368 Research Park Dr.
    • Cash: $100 in-person at Beavercreek City Hall, 1368 Research Park Dr.
    • Credit Card: $100 via telephone or in-person at Beavercreek City Hall, 1368 Research Park Dr. (Additional $3 service fee applies).

I also give the City of Beavercreek permission to use the service member’s image and likeness on the printed banner, city website, social media platforms, and publications. This signed agreement states that I have read and agreed to all terms set forth in the City of Beavercreek’s Veteran Banner Program Policy.
